Where pets should go in case Mount Redoubt decides to blow

As many people take precautions in case Mount Redoubt does erupt and sends a thick storm of ash falling from the skies, so too should pet owners take steps to protect their furry friends.

"The main thing is to keep them out of it as much as possible," said Tabitha Perkovich, a veterinarian at Twin Cities Veterinary Clinic in Soldotna.
Like humans, the lungs of animals can be adversely affected if they breathe in ash. Obstructive airway diseases -- such as pneumonia, asthma or emphysema -- could develop as a result of a significant ash-fall event.
"You can't really get pets to wear masks, so you have to limit their exposure by keeping them indoors, only going in-and-out to do their business," Perkovich said.

All steps should be avoided to keep pets from ingesting ash while eating and drinking, as well.

"Bring them into the garage to eat and drink. Don't offer them food and water outside," she said.

Precautions should be taken to protect the eyes of animals too, according to Mary Huhndorf, also a veterinarian at Twin Cities Veterinary Clinic.
"Opthalmic ointments can be put in to protect the eyes from corneal abrasions," she said.

Pets will undoubtably pick up some ash on their coats and feet while outdoors, and Huhndorf said this should be removed immediately upon returning indoors.

"They can be vacuumed or brushed, and extra effort should be made to keep the face clean, but don't use anything wet to wipe them, it'll just make a cement," she said.

Most of these recommendations shouldn't be tough for people with indoor dogs and cats, or even a small number of outdoor pets. But, for sled dog kennels, or the owners of livestock, this may be a bit more challenging.

"I've given a lot of thought to this issue," said Sterling musher Mitch Seavey.
He said he has contemplated loading his dogs and heading out of town to avoid an ash-fall event, but until the volcano does blow and he knows which way the wind may blow, he is reluctant to take off. Instead, he said he will likely take steps to protect his dogs at home.
"I've got a trailer and two dog trucks and shops and garages, so my plan is to put them in there," he said.

Other sled dog owners are encouraged to do the same, and Perkovich and Huhndorf said those that have more dogs than they do places to house them out of the ash, may want to invest in pet travel kennels, so that dogs may be able to be indoors in these containers.

Makeshift shelters -- such as made with tarps, Tyvek or other materials -- can also be used outdoors to help protect animals too large or too wild to be brought indoors. For sled dogs, this shelter could be constructed around a drop chain, anchored on each end.

"Even if they could put up a Visqueen shelter, around and over the animals, it would help cut down on the percent of particles they may be exposed to," Perkovich said.

For pet owners that want to bring the animals indoors in kennels, but have nowhere to put the kennel, the Kenai Animal Control Shelter said it would be willing to temporarily take in a limited number of pets, but pet owners should only utilize them as a last resort.

"If someone had nowhere else to go, we could hold some animals, but space is limited. At a push, we could do 24 dogs and put some crates in the garage. The same with cats," said Patricia Stringer, Kenai's Chief Animal Control Officer.
Soldotna residents will not have the same option.

"I have not been given that direction from my management," said Marianne Clark, manager of Soldotna's Animal Control Shelter.

As such, she said, "Pet owners should have some kind of provisions to bring their animals indoors."

The Kenai Peninsula Borough Office of Emergency Management also has a list of emergency precautions pet owners can take, and -- while they are not volcano specific -- they may serve as good guidelines for what to have on hand for pets in the event the water, power and avenues of transportation become unavailable for days to weeks.

Bipolar Disorder and Sleep
How many hours do you sleep on average at night, and what is the quality of your sleep? are two of the first questions I ask every patient on the initial interview and all subsequent follow-up visits. While the hypomanic usually gloats over how little sleep he needs, getting by on 3 to 4 hours a night, the lack of quality sleep can wreak havoc on his mood and decision-making abilities. Sleep deprivation results in feelings of malaise, poor concentration, and moodiness, and even accidental deaths.
In a revealing sleep study published in the September 2005 issue of the Journal of the American Medical Association, Judith Owens, MD, and her team of researchers from Hasbro Childrens Hospital in Providence, Rhode Island, followed 34 pediatric residents from Brown University over the course of 2 years to compare post-call performance to performance after drinking alcohol. During this time, the residents were tested under light call (1 month of daytime duty with no overnight shift, or about 44 hours of work per week) and heavy call (overnight duty every fourth night with an average of 90 hours of work a week). The residents performed computer tasks to gauge their attention and judgment after their light call (after consuming alcohol) and heavy call shifts (with placebo). The residents who were on heavy call and had not ingested alcohol performed worse on the computer tests than those doctors who had taken alcohol and were on light call. Dr. Owens concluded that the residents were so sleep-deprived that they didnt recognize that their own judgment was impaired.

Insomnia
How is your sleep? Do you have difficulty falling asleep? Or do you toss and turn most of the night until you fall into a deep sleep just hours before the alarm goes off? A person suffering from insomnia has difficulty initiating or maintaining normal sleep, which can result in non-restorative sleep and impairment of daytime functioning. Insomnia includes sleeping too little, difficulty falling asleep, awakening frequently during the night, or waking up early and being unable to get back to sleep. It is characteristic of many mental and physical disorders. Those with depression, for example, may experience overwhelming feelings of sadness, hopelessness, worthlessness, or guilt, all of which can interrupt sleep. Hypomanics, on the other hand, can be so aroused that getting quality sleep is virtually impossible without medication. In a study at the University of Oxford in the United Kingdom, Allison G. Harvey, PhD, and colleagues in the department of experimental psychology determined that even between acute episodes of bipolar disorder, sleep problems were still documented in 70 percent of those who were experiencing a normal (euthymic) mood at the time. These normal-mood patients with bipolar disorder expressed dysfunctional beliefs and behaviors regarding sleep that were similar to those suffering from insomnia, such as high levels of anxiety, fear about poor sleep, low daytime activity level, and a tendency to misperceive sleep. Dr. Harvey concluded that even when the bipolar patients were not in a depressive, hypomanic, or manic mood state, they still had difficulty maintaining good sleep.
Delayed Sleep Phase Syndrome
This is the most common circadian-rhythm sleep disorder that results in insomnia and daytime sleepiness, or somnolence. A short circuit between a persons biological clock and the 24-hour day causes this sleep disorder. It is commonly found in those with mild or major depression. In addition, certain medications used to treat bipolar disorder may disrupt the sleep-wake cycle. I often recommend chronotherapy to patients. This therapy an attempt to move bedtime and rising time later and later each day until both times reach the desired goal is often used to adjust delayed sleep phase syndrome. To adjust the delayed sleep phase problem, sleep specialists might also use bright light therapy or the natural hormone melatonin, particularly in depressed patients.
REM Sleep Abnormalities
REM sleep abnormalities have been implicated by doctors in a variety of psychiatric disorders, including depression, posttraumatic stress disorder, some forms of schizophrenia, and other disorders in which psychosis occurs. Special tests, called sleep electroencephalograms, record the electrical activity of the brain and the quality of sleep. From these tests, we know that in people who are depressed, NREM sleep is reduced and REM sleep is increased. Most antidepressant medications suppress REM sleep, leading some researchers to believe that REM sleep deprivation relates to an improvement in depressive symptoms. Yet Wellbutrin XL, a common antidepressant, and some older medications used to treat depression do not suppress REM sleep. Researchers are therefore still trying to determine the connection between the REM sleep mechanism and depression.
Irregular Sleep-Wake Schedule
This sleep disorder is yet another problem that many with Bipolar II experience and in large part results from a lack of lifestyle scheduling. The reverse sleep-wake cycle is usually experienced by bipolar drug abusers and/or alcoholics who stay awake all night searching for similar addicts and engaging in drug-seeking behavior, which results in sleeping the next day. This sleep disruption and irregularity make it much more difficult for the bipolar patients physician to treat him or her with conventional medications and adjunctive cognitive therapy. In most cases, the patient needs to acknowledge the drug-seeking behavior and get involved in a recovery program such as Alcoholics Anonymous, Cocaine Anonymous, or other group. Talk therapy with a psychologist is beneficial to many patients as they seek to change destructive lifestyle habits and learn new behaviors that will help them adhere to a more normal sleep-wake schedule.

The process in witch the hormone stops being produce happens gradually. Another variable is how active we are. This process is very specific to each woman maybe being very easily to go thru or even on the opposite side with a lot of severe complications. Some people are more active than others.

In an age of consumer abundance, all these factors lead to an almost inevitable epidemic of excess. Women may experience abnormal bleeding, flair ups or different timing on their periods. We've literally forgotten how to listen to the signals our very efficient bodies give us when we need refuelling or when the tank is full. Scientists are still trying to determine the causes that make this symptoms so erratic. Many obese people eat before they get hungry unless they're on a diet when they doggedly ignore their hunger. The symptoms will eventually stop and completely disappear along with the woman's ability to create more eggs or estrogen, thus the capacity of procreation. A lot of this unhealthy, self-esteem sapping behaviour is habitual and seated deeply within the subconscious, outside of our conscious control or rational powers of reason.

The age in at witch the menopause starts tends to be the same in the majority of women. Thus it is easy to be overweight, believe you don't eat very much and be genuinely baffled as to how this could have come about.

The process of deconstructing misleading beliefs and expectations starts with our emotional state. But of course not all of them are complying with this rule. Many people think they can only be happy when they are thin when, in fact, creating a happy, positive and motivated state of mind is the first step towards a slimmer waistline rather than the end result.

It is then possible, using the 'modelling' principles of neuro-linguistic programming (NLP), to start thinking, feeling and behaving like a person with a healthy body and lifestyle. NLP is a powerful force for change which enables people to see their world differently and work towards realistic goals using all the skills and resources available to them. New eating patterns and a routine increase in physical activity which take into account personal preferences and lifestyle choices can be easily assimilated on a permanent basis. To be shore she must in any case check up with a doctor. It's this that is the key to long term weight control.

Blood test shouldn't relied on to tell if the onset of the menopause has began. A combined programme of hypnotherapy and NLP provides the awareness, desire and capability to stay slim for life.

CHICAGO, Wed Jan 21,(bdnews24.com/Reuters) - The popular antidepressant Lexapro showed promise at easing anxiety symptoms in older adults, but the effect was "modest" and would need to be studied further, US researchers said on Tuesday.
They said antidepressants like Lexapro, made by Forest Laboratories Inc and known generically as escitalopram, may be useful as a new treatment option for older adults with generalized anxiety disorder, a disabling condition that can also cause muscle tension, insomnia and fatigue.
"We found improvements not only in anxiety and precise of bad news but also in functioning," Dr. Eric Lenze of Washington University School of Medicine in St. Louis, whose study appears in the Journal of the American Medical Association, said in a statement.
He said many people who took the drug were better able to carry out their daily activities. But for most people, he said, the drug alone is likely not enough.
"Overall the benefits were fairly modest," Lenze said in a telephone interview. "It will comfort some mortals a lot. Most people will probably need some sort of combination treatment."
Lenzes team did uncover an unexpected benefit: the drug helped people get their blood pressure under control.
"That suggests there can be some lasting-term clean bill benefits in treating anxiety in that older age group," Lenze said.
While the study looked only at Lexapro, Lenze thinks the benefits would extend to all antidepressants in the class, which are known as selective serotonin reuptake inhibitors or SSRIs.
Older patients with anxiety typically get no treatment or are prone sedatives such as Valium or Xanax. And while these drugs can relieve anxiety, they can also impair thinking ability and can even lead to falls.
In the study, Lenze and colleagues evaluated 177 people aged 60 or older with generalized anxiety disorder who got either Lexapro or a dummy pill for 12 weeks.
They found 69 percent of patients got better, compared with 51 percent of those whose symptoms improved unaffectedly by taking a placebo. Those who took the drug showed greater improvement in both anxiety symptoms and in social functioning.
In a more conservative analysis that included people who had dropped out of the study, the drug showed no benefit over the placebo arm. Side effects in the drug arm included fatigue, insomnia and urinary symptoms.
Lenze is now studying the long-term effects of treatment and is also studying the drug in combination with a goal-oriented type of talk therapy called cognitive behavioral therapy.
Forest, which faces the loss of patent protection for Lexapro in 2012, supplied drugs for the study. It was funded by the National Institutes of Hardihood.

Two Surgeons from the UK, Dr Ghassan Abu Sittah and Dr Swee Ang, managed to get into Gaza during the Israeli invasion. Here they describe their experiences, share their views, and conclude that the people of Gaza are extremely vulnerable and defenseless in the event of another attack. The wounds of Gaza are deep and multi-layered. Are we talking about the Khan Younis massacre of 5,000 in 1956 or the execution of 35,000 prisoners of war by Israel in 1967? Yet more wounds of the First Intifada, when civil disobedience by an occupied people against the occupiers resulted in massive wounded and hundreds dead? We also cannot discount the 5,420 wounded in southern Gaza alone since 2000. Hence what we are referring to below are only that of the invasion as of 27 December 2008.
Over the period of 27 December 2008 to the ceasefire of 18 Jan 2009, it was estimated that a million and a half tons of explosives were dropped on Gaza Strip. Gaza is 25 miles by 5 miles and home to 1.5 million people. This makes it the most crowded area in the whole world. Prior to this Gaza has been completely blockaded and starved for 50 days. In fact since the Palestinian election Gaza has been under total or partial blockade for several years.
On the first day of the invasion, 250 persons were killed. Every single police station in Gaza was bombed killing large numbers of police officers. Having wiped out the police force attention was turned to non government targets. Gaza was bombed from the air by F16 and Apache helicopters, shelled from the sea by Israeli gunboats and from the land by tank artillery. Many schools were reduced to rubble, including the American School of Gaza, 40 mosques, hospitals, UN buildings, and of course 21,000 homes, 4,000 of which were demolished completely. It is estimated that 100,000 people are now homeless.
Israeli weapons
The weapons used apart from conventional bombs and high explosives also include unconventional weapons of which at least 4 categories could be identified.
Phosphorus Shells and bombs
The bombs dropped were described by eye witnesses as exploding at high altitude scattering a large canopy of phosphorus bomblets which cover a large area.
During the land invasion, eyewitnesses describe the tanks shelling into homes first with a conventional shell. Once the walls are destroyed, a second shell - a phosphorus shell is then shot into the homes. Used in this manner the phosphorus explodes and burns the families and the homes. Many charred bodies were found among burning phosphorus particles.

One area of concern is the phosphorus seems to be in a special stabilizing agent. This results in the phosphorus being more stable and not completely burning out. Residues still cover the fields, playground and compounds. They ignite when picked up by curious kids, or produce fumes when farmers return to water their fields. One returning farming family on watering their field met with clouds of fumes producing epistaxis. Thus the phosphorus residues probably treated with a stabilizer also act as anti-personnel weapons against children and make the return to normal life difficult without certain hazards.
Surgeons from hospitals are also reporting cases where after primary laparotomy for relatively small wounds with minimal contamination find on second look laparotomy increasing areas of tissue necrosis at about 3 days. Patients then become gravely ill and by about 10 days those patients needing a third relook encounter massive liver necrosis. This may or may not be accompanied by generalized bleeding, kidney failure and heart failure and death. Although acidosis, liver necrosis and sudden cardiac arrest due to hypocalcemia are known to be a complication of white phosphorus it is not possible to attribute these complications as being due to phosphorus alone.
There is real urgency to analyze and identify the real nature of this modified phosphorus as to its long term effect on the people of Gaza. There is also urgency in collecting and disposing of the phosphorus residues littering the entire Gaza Strip. As they give off toxic fumes when coming into contact with water, once the rain falls the whole area would be polluted with acid phosphorus fumes. Children should be warned not to handle and play with these phosphorus residues.
Heavy Bombs
The use of DIME (dense inert material explosives) were evident, though it is unsure whether depleted uranium were used in the south. In the civilian areas, surviving patients were found to have limbs truncated by DIME, since the stumps apart from being characteristically cut off in guillotine fashion also fail to bleed. Bomb casing and shrapnel are extremely heavy.
Fuel Air Explosives
Bunker busters and implosion bombs have been used . There are buildings especially the 8 storey Science and Technology Building of the Islamic University of Gaza which had been reduced to a pile of rubble no higher than 5-6 feet.
Silent Bombs
People in Gaza described a silent bomb which is extremely destructive. The bomb arrives as a silent projectile at most with a whistling sound and creates a large area where all objects and living things are vaporized with minimal trace. We are unable to fit this into conventional weapons but the possibility of new particle weapons being tested should be suspected.
Executions
Survivors describe Israeli tanks arriving in front of homes asking residents to come out. Children, old people and women would come forward and as they were lined up they were just fired on and killed. Families have lost tens of their members through such executions. The deliberate targeting of unarmed children and women is well documented by human right groups in the Gaza Strip over the past month.
Targeting of ambulances
Thirteen ambulances had been fired upon killing drivers and first aid personnel in the process of rescue and evacuation of the wounded.
Cluster bombs
The first patients wounded by cluster were brought into Abu Yusef Najjar Hospital. Since more than 50% of the tunnels have been destroyed, Gaza has lost part of her lifeline. These tunnels contrary to popular belief are not for weapons, though small light weapons could have been smuggled through them. However they are the main stay of food and fuel for Gaza.
Palestinians are beginning to tunnel again. However it became clear that cluster bombs were dropped on to the Rafah border and the first was accidentally set of by tunneling. Five burns patients were brought in after setting off a booby trap kind of device.
Death toll
As of 25 January 2009, the death toll was estimated at 1,350 with the numbers increasing daily. This is due to the severely wounded continuing to die in hospitals. 60% of those killed were children.
Severe injuries
The severely injured numbered 5,450, with 40% being children. These are mainly large burns and polytrauma patients. Single limb fractures and walking wounded are not included in these figures.
Through our conversations with doctors and nurses the word holocaust and catastrophe were repeatedly used. The medical staff all bear the psychological trauma of the past month living though the situation and dealing with mass casualties which swamped their casualties and operating rooms. Many patients died in the Accident and Emergency Department while awaiting treatment. In a district hospital, the orthopaedic surgeon carried out 13 external fixations in less than a day.
It is estimated that of the severely injured, 1,600 will suffer permanently disabilities. These include amputations, spinal cord injuries, head injuries, large burns with crippling contractures.
Special factors
The death and injury toll is especially high in this recent assault due to several factors:
No escape: As Gaza is sealed by Israeli troops, no one can escape the bombardment and the land invasion. There is simply no escape. Even within the Gaza Strip itself, movement from north to south is impossible as Israeli tanks had cut the northern half of Gaza from the south. Compare this with the situation in Lebanon 1982 and 2006, when it was possible for people to escape from an area of heavy bombardment to an area of relative calm - there was no such is option for Gaza.
Gaza is very densely populated. It is eerie to see that the bombs used by Israel have been precision bombs. They have a hundred percent hit rate on buildings which are crowded with people. Examples are the central market, police stations. Schools, the UN compounds used as a safety shelter from bombardment, mosques (40 of them destroyed), and the homes of families who thought they were safe as there were no combatants in them and high rise flats where a single implosion bomb would destroy multiple families. This pattern of consistent targeting of civilians makes one suspect that the military targets are but collateral damage, while civilians are the primary targets.
The quantity and quality of the ammunition being used as described above.
Gaza's lack of defense against the modern weapons of Israel. She has no tanks, no planes, no anti-aircraft missiles against the invading army. We experienced that first hand in a minor clash of Israeli tank shells versus Palestinian AK47 return fire. The forces were simply unmatched.
Absence of well constructed bomb shelters for civilians. Unfortunately these will also be no match for bunker busters possessed by the Israeli Army.
Conclusion
Taking the above points into consideration, the next assault on Gaza would be just as disastrous. The people of Gaza are extremely vulnerable and defenseless in the event of another attack. If the International Community is serious about preventing such a large scale of deaths and injuries in the future, it will have to develop a some sort of defense force for Gaza. Otherwise, many more vulnerable civilans will continue to die.
